- Develop a hybrid analog system for chronic care patients who are not digital savvy and can’t provide data using the health application.
- Build an independent application for Care Management Agents (CMA) to access this information.
- Build workflow between clinicians and CMAs to provide personalized care messages over phone/email or face to face appointment.
SOLUTIONS
- Created Digital solution to read scanned sheets of patients’ information by care management agents.
- Built Angular.JS framework based user interface for care management agents that is synced with EHR system and health application.
- Have set up support services and communication systems for hybrid analog model between patients, clinicians and agents.
